The 31st Annual Seminole Memorial Soccer tournament.
This packet contains all the important information that you will need to know about the tournament for your team. Please read through the packet carefully.
Official roster form
Teams may include up to 4 guest players. A player may play for one team only during the tournament. Teams, players, and coaches must be in good standing and be registered with their State or National Association. Players and coaches must have current, valid, and verified player passes which have been laminated with a current photo on the back.
Proper travel procedures must be followed. You will need an FYSA or US Club Soccer Official State roster.
If you are adding any guest players they must fill out a guest player form, the guest player form is located on the secure part of the FYSA website. The coach is not able to access that form. Only the club registrars have access to that form. Please inform these coaches that they must go to their club registrar to obtain the Guest Player Form.
This form(s) must be attached to the Official State roster. You must make one copy of each guest player form and attach it to each copy of the roster.
You will need a total of 5 bundles, each of them containing a roster and any guest player forms.
Medical Release Forms
FYSA requires that during check in we verify that each player has a properly filled out Medical release form that is signed by a parent and notarized. Please be sure all are in order for each team member prior to check in at the coaches meeting. These forms expire one year after the date in which they are notarized, please make sure that yours are up to date. You can use the ones that you have been using this past season as long as they are current.

Team check in
Team Check-in place on Friday May 22nd 7:00-8:30 at the Hilton Garden Inn 705
Currency Cr Lake Mary Fl 32746 407-531-9900. In order to shorten time required
for check-in, please have your roster, medical release forms, and player passes
in alphabetical order. There will be no coaches meeting. At the mandatory team
registration, coaches must submit the original plus 4 copies of the team roster
with guest player forms attached. Additionally each team must provide a notarized
Medical Release From for each player and present current, valid and verified
laminated player passes.

10 days prior to tournament date.
We will not be sending schedules out to each team.
It is each coach's responsibility to check the site daily for your schedule as well as for future changes.
